Each School-Based Health Center
has a health care team who support your care.
RIGHTS
At the school–based health center, you will get from us:
Private care and treatment
- physical exams for camp, sports, college and other things
- eye and ear check–ups
- minor injuries and prescriptions
- protection from getting pregnant or sexually transmitted infections
- sexual health education
- mental health concerns
Respect from all staff,
Whether you are:
- a girl or a boy
- gay, lesbian, straight or unsure
- insured or not
- disabled
Whatever your:
- age
- skin color
- religion or beliefs
- language
Choices about your care and treatment
- you can say NO at any time
- you can say STOP at any time during your exam
- you can ASK at any time for someone to be with you during an exam
Information you can understand
- help if you don’t speak English
- help if you have a disability of any kind
In order to provide you with the best care, the law says we may let some professionals read your health information. They are the only ones who see it.
Please ask a staff person to explain this.
RESPONSIBILITIES
Show up on time for appointments.
Contact the clinic if you need to cancel your appointment.
Report any health insurance or changes in your insurance.
Tell us if your address or phone number changes.
Be sure you understand how to take any medications.
Tell us before you stop taking any medications or are having problems.
Make sure you know what you are supposed to do once you leave the clinic.
Know when to come back to the health center for follow-up treatment or know where to go or who to call for other services.
Ask us any questions about anything you do not understand.
If possible, involve your parents or guardians about the care you receive at the health center.
We can help you talk to them and provide you with information to take to them.
